چکیده

BACKGROUND To determine whether treatment with antimicrobial photodynamic therapy (aPDT) as an adjunct to scaling and root planing (SRP) improves periodontal clinical and glycemic outcomes in chronic periodontitis patients (CP) with type 2 diabetes mellitus (DM). METHODS Databases (MEDLINE via PubMed; EMBASE; Cochrane Central Register of Controlled Trials and Cochrane Oral Health Group Trials Register databases) were searched up to and including October 2016. The addressed PICO question was: "What are the effects of aPDT as an adjunct to SRP in terms of periodontal and glycemic outcomes as compared to SRP alone in individuals with DM?" RESULTS Four randomized clinical trials were included in the present review. All studies reporting clinical periodontal and metabolic parameters, showed that aPDT was effective in the treatment of CP in DM subjects at follow up. Considering the effects of aPDT as an adjunct as compared to SRP alone on clinical signs of CP in DM subjects, no difference could be observed for all evaluated parameters (PD: Z=-0.61, P=0.54; CAL: Z=0.27, P=0.78; HbA1c: Z=0.138, P=0.89) CONCLUSION: It remains debatable whether aPDT is effective as an adjunct to SRP than SRP alone in patients having CP with DM, given that the scientific evidence is weak. In terms of periodontal parameters and glycemic levels, aPDT does not provide additional benefit in the treatment of CP in DM patients. However, further randomized clinical trials with standard laser parameters and long follow up periods are warranted to study periodontal and glycemic outcomes in this regard.

The Effect of Resveratrol Supplementation in Adjunct with Non-surgical Periodontal Treatment on Blood Glucose, Triglyceride, Periodontal Status and Some Inflammatory Markers in Type 2 Diabetic Patients with Periodontal Disease

Background and Objectives: Diabetes mellitus and periodontal disease are two chronic and common diseases with close relationship together affecting public health and quality of life. The aim of this study was to investigate the effect of resveratrol supplementation in adjunct with non-surgical periodontal treatment (NST) on blood glucose, triglyceride, periodontal status and inflammatory marker...

Antimicrobial photodynamic therapy combined with periodontal treatment for metabolic control in patients with type 2 diabetes mellitus: study protocol for a randomized controlled trial

BACKGROUND The relationship between diabetes mellitus (DM) and periodontal disease is bidirectional. DM is a predisposing and modifying factor of periodontitis, which, in turn, worsens glycemic control and increases proteins found in the acute phase of inflammation, such as C-reactive protein. The gold standard for the treatment of periodontal disease is oral hygiene orientation, scaling and pl...

Effects of Non-Surgical Periodontal Therapy on Gingival Crevicular Fluid Levels of Interleukin-17 and Interleukin-23 in Patients with Periodontitis: A Clinical Trial

Background and Aim: Considering the critical role of inflammatory responses in periodontal disease, knowledge about the immune system and cytokines seems nec-essary. The aim of this study was to evaluate the level of IL-23 and IL-17 in gingival crevicular fluid (GCF) of patients with chronic and aggressive periodontitis, before and after non-surgical periodontal therapy. Materials and Methods:...

Efficacy of insulin targeted gene therapy for type 1 diabetes mellitus: A systematic review and meta-analysis of rodent studies

Objective(s): Diabetes mellitus (DM) is a major worldwide public health challenge, for which gene therapy offers a potential therapeutic approach. To date, no systematic review or meta-analysis has been published in this area, so we examined all relevant published studies on rodents to elucidate the overall effects of gene therapy on bodyweight, intraperitoneal glucose...

Short-term effects of photodynamic therapy on periodontal status and glycemic control of patients with diabetes.

BACKGROUND Periodontitis is a major cause of tooth loss among adults. Several studies have shown a possible systemic impact of periodontal infection, including poor glycemic control in patients with diabetes. Recently, photodynamic therapy (PDT) was used to successfully treat periodontal infection. PDT provides a broad spectrum antimicrobial efficacy with no local or systemic side effects. The ...
